Community Inclusion

Description:

The purpose of this course is to introduce readers to Polk County Health Services' Community Inclusion outcome for people with disabilities. The course describes the values that led to the Community Inclusion outcome, how it is currently defined, suggestions for documenting when people meet the outcome and evaluating that documentation.

Objecives

On completing this course, the learner should be able to

 

  • Describe the Community Inclusion Outcome
  • Explain why Community Inclusion is important for people with disabilities
  • Identify the three Community Inclusion Outcome categories
  • Identify the criteria for eligible Community Inclusion Outcome activities
  • Explain how the PCHS Community Inclusion Outcome is determined
  • Evaluate how Community Inclusion is documented and reported
