How is the Community Inclusion outcome measured and defined?
The Community Inclusion outcome is measured as the percent of participants who exhibit ongoing involvement in community inclusion activities during a given fiscal year. Deciding whether an individual has met the Community Inclusion outcome (as defined by PCHS) is a multistep process.
Step 1: The activity must qualify as a Community Inclusion activity.
Step 2: The activity must fall within one of three Community Inclusion categories.
Step 3: To be "ongoing," the individual must have at least three Community Inclusion activities within one category during the fiscal year.
