What is the Community Inclusion outcome?
To monitor and promote the full participation in their communities of people with mental health, developmental and intellectual disabilities who are receiving services, PCHS has developed a Community Inclusion Outcome as part of their annual evaluations. PCHS's expectation is that participants will engage in integrated, community-based activities of their choosing. The outcome is intended to promote the removal of barriers to community activities and to encourage people with disabilities to be a part of their communities.
