The following is NOT sufficient documentation of Community Inclusion activities:

Met Jane at her home. … Jane is looking forward to taking her children to upcoming Halloween events: Ankeny Pumpkin Party (October 21), The Great Pumpkin Party at Valley Church (October 22), and Grimes Halloween Fest (October 25). Jane is choosing to attend these events (person-directed), all are in the community (community-based) and open to the public (integrated). Jane says that her children usually enjoy dressing in their costumes and getting free candy. They are all looking forward to attending these and similar holiday events. Thus, Jane meets criteria for the Community Inclusion outcome.

This case note documents intent to participate in activities but not actual participation (see the text in bold). To qualify as a Community Inclusion activity, documentation needs to be completed after the individual has participated to document their participation. The reviewer should continue reading in the file as staff may have followed up with the participant after October 25 to find out if the participant actually attended these activities and then documented the participant's attendance for Community Inclusion. Once attendance at these activities has been documented, then an event can be entered into PolkMIS for Community Inclusion.
