The Community Inclusion outcome defines goals based on policy where providers encourage and support participants to reach out and participate in their community. One of the purposes of PolkMIS events and the file review is to understand the extent to which barriers are being removed and participants are taking steps to be included. The task of the reviewer is to determine the quality of the data, in the numbers (PolkMIS) as verified by case notes. Thus, it is important that PolkMIS data and case notes be both accurate and consistent.

Therefore, events recorded in PolkMIS should accurately reflect participant activity. Case files should not only accurately reflect participant activity, but should also provide support for their accuracy. For Community Inclusion this support is shown by including dates, names, and types of community activities. When the documentation from the files align with the events in PolkMIS, both in adequately documenting the events and in consistency with the events, the reviewer can be assured that Community Inclusion data is accurate.
