Overreporting occurs when a participant has a Community Inclusion event in PolkMIS but sufficient documentation is not found in the file to support that the participant has met the Community Inclusion outcome. Overreporting indicates that the agency may have claimed an outcome that they have not documented. This type of error may result in overreporting of agency performance of Community Inclusion. This error can also occur when a participant meets community inclusion criteria in reality, and this is reported in PolkMIS, but is not documented sufficiently in the case file.

Underreporting occurs when a participant does not have a Community Inclusion event in PolkMIS but sufficient documentation is found in the file that the participant has met the criteria for the Community Inclusion Outcome. Underreporting indicates that the agency has not claimed outcomes that they may have achieved. This type of error results in underreporting of agency performance of Community Inclusion.
