The following note sufficiently documents a Community Inclusion Outcome:

F:F with client / December 20, 2015

Met with Joe at his home. … Joe mentioned he continues to attend Alcoholics Anonymous meetings in the basement of a local church each week. These meetings meet criteria in the cultural category for Community Inclusion; it is his choice to attend, they are held in the community and open to the public. He has attended more than three in the past month alone. Joe has met the Community Inclusion outcome for the year.

Joe's attendance at Alcoholics Anonymous qualifies as a Community Inclusion activity as it is person-directed, community-based and integrated. Although the reporter has not documented the exact dates, this case note documents the time span in which the individual participated in the activities as November and December of the current year (i.e., "three in the past month alone") to document that it is within the fiscal year. The three activities all fall within the cultural category.
