Discrepancy: Example 5
The following note does NOT sufficiently document the Community Inclusion Outcome:
F:F with client / December 20, 2015
Met with Jackie at her home. … Jackie was excited to tell me that she attended Bible study at her local church two times in November. She also visited Jolly Holiday Lights this last weekend with friends. Jackie chose these activities, they were held in the community and integrated. She meets the Community Inclusion outcome for the year.
As the writer indicates, the three activities meet the criteria for Community Inclusion activities. Although there are three activities, they do not demonstrate on-going activity because she has not completed three activities within one category. Two (bible study) are spiritual while the other (visiting a local attraction) is civic. The reviewer should continue reading in the file as staff may have followed up with Jackie later in the fiscal year to document participation in additional activities. If she attends a third spiritual or two additional civic activities, then she would meet outcome criteria.
