The following qualifies as a Community Inclusion activity:

Victoria tells her Supported Community Living staff that she wants to attend a free concert in the park. One of Victoria’s goals is to be more active in the community, but she needs staff to help support her. Staff attend the event with her as part of her service plan. Victoria chooses to attend the concert (person-directed) at a local park (community-based), and the event is open to the public (integrated). Thus, it would qualify as one of the three Community Inclusion activities for the year.


The following does NOT qualify as a Community Inclusion activity:

Victor attends a matinee at the Des Moines playhouse as part of Life Club. Life Club is an Easter Seals’ day habilitation program. The activity is at a local theater (community-based) and the event is open to the public (integrated). However, it is a group activity organized by the day habilitation program and not individually chosen by the participant. Although Victor has the option to attend the matinee with Life Club or not attend Life Club that day, it is the organization (Life Club) that is choosing which activity to offer (not person-directed). Thus, it would not meet criteria as a Community Inclusion activity.
